Topic: Support for Chronic Illness---Disease Trajectory and Person-Centered Care - This presentation will cover what disease trajectory means to the patient/family, goals of care and what is palliative care. It will touch on the meaning and importance of advance care planning and who should be involved. It also includes determining what is done (or not), the impact on the patient receiving palliative care and anticipatory guidance.
Speaker: Mary Grant, RN, MS, ANP has been a nurse for over 40 years and a nurse practitioner for 34 years. She has worked in primary care with physicians and at a large Portland gastroenterology practice. Mary has been with Providence Connections Palliative Care Consultation Service for more than eight years and is the lead nurse practitioner. She primarily focuses on out-patient visits in homes or wherever the patient calls home. She also assists the Providence Home Health palliative care nurses with education and patient management.
