Providence Home and Community Care – Providence Home and Community Care provides Home Health, Hospice, Pharmacy Services and Home Medical Equipment services in Oregon and southwest Washington. All services are state-licensed and/or accredited by The Joint Commission. 

Providence Home and Community Care offers care and support to individuals at all stages of life. The focus is especially on frail older adults and people diagnosed with serious, chronic illness to ensure that individuals are safe and nurtured wherever they are in life. We offer centralized access for convenient, immediate and multi-service referrals. Providence Home and Community Care access teams are capable of managing all referrals from health care providers, resulting in continuing care for patients.
